Intake and Assessment

Our Intake and Assessment Team is comprised of Georgia-licensed social workers, psychologists and psychology technicians, registered nurses and a physician. Team members work together to:

• Conduct eligibility screenings for people seeking developmental disability services.
• Schedule and conduct assessments and annual updates for people receiving services, and produce a summary of findings for each assessment completed.
• Participate in the development of Individual Support Plans by suggesting appropriate services, supports and other recommendations.
• Ensure continued eligibility for services through level of care reviews and approvals.
• Provide pre-approved consultation, technical assistance and training to providers and staff to effect best practices in service delivery and implementation of ISPs.
• Assure that consumers are afforded all due process and appeals rights.
• Maintain documentation and tracking of all screening/evaluation functions.
