How to Apply for Services

If you or a family member has a developmental disability and you would like to apply for services from the State, there are several ways to complete the application process. First, be sure to read the Guide to Georgia's Services that is located on this web site. It should answer lots of questions about services in Georgia.

You may come to any Columbus Community Services offices listed on this web site. The one nearest you may be located by locating your county on the map or on the county list to see which is your region. While at the office, you will be provided assistance with filling out the application form.

You may call or write to any of Columbus Community Services' offices and we will mail you the Application for Service form and the Guide to Services. The phone numbers and addresses for the offices are located on this web site.

Another way to apply is to read the Guide to Services from this web site and then print the Application form posted on this site. You may either mail these to Columbus Community Services or bring them to your nearest office.

Our goal is to make this an uncomplicated experience that meets your unique needs. You may call the 1-800-579-7609 number at any time for further assistance. Once we receive your application, an appointment will be scheduled to complete a preliminary eligibility screening within fourteen business days. We will make every effort to schedule a time and place for the meeting that are both convenient and accessible to the person and family applying for services.
